When I've purchased packages of pictures from most any photographer in the past, there has been an option for a "locket" picture. I've never actually ordered one but I'll bet most any photographer would offer that.
If you already have the picture that you want to put in the locket, the Kodak Photo machines that they have at Walgreens or Target might be able to shrink it down that small. Another option is Wolf Camera next to the west side Kroger - they may be able to do something like that.
